Rome is indeed run down, surprisingly so. But cannot be skipped. You have to visit it at least once to see living history of Western Europe. Florence is amazing but packed with tourists. Milan. Sienna, Venice. The list goes on.

Yes, Aosta Valley is awesome (Val D'Aosta), Mt Blanc/Coumayeur on one end and Cervinia/Matterhorn on the other end with many side valley's in between.

Dolomite area, just north of Naples is gorgeous if your into mtns. Naples itself is tourist shithole.

Down south, Amalfi Coast, Positano and Sorrento are very good, but skip Capri, another tourist shithole, nearby Ischia is where the locals go.

Milan didn't do anything for me, but the nearby lakes region/Como is nice.
