“Bottom surgery”, or the gender affirming procedure for male-to-female transgender persons is called vaginoplasty. The male genitalia is repurposed to form a vagina and vulva. There is no penis or testicles remaining after the procedure. Unless you’re very wealthy, it’s unlikely that you could find a surgeon that would perform the procedure on someone that isn’t MtF transgendered, and well into their transition. With enough money and a plane ticket to Thailand, maybe.

From what I understand, recovery from vaginoplasty is no picnic, as you could imagine. There is a simpler version of the procedure known as zero-depth vaginoplasty that has fewer complications and shorter recovery, but doesn’t afford vaginal penetration.
